Georgia's CCSP and SOURCE programs operate under the Elderly and Disabled Waiver Program and are designed to help eligible individuals receive services in their homes and communities rather than requiring nursing-facility placement.
*Eligibility, authorization, covered services, and availability are determined by the applicable Georgia Medicaid program and the individual's approved plan of care. DenWard Healthy Living does not guarantee eligibility or authorization for Medicaid services.
Community Care Services Program
The Community Care Services Program (CCSP) is part of Georgia's Elderly and Disabled Waiver Program. CCSP helps eligible individuals receive home- and community-based services as an alternative to nursing-home placement. Depending on an individual's approved plan of care, services may include personal care, adult day health, meals, respite, and other supportive services. DenWard Healthy Living is committed to helping eligible individuals receive dependable services while remaining connected to their homes, families, and communities.
Service Options Using Resources in a Community Environment
SOURCE is another component of Georgia's Elderly and Disabled Waiver Program. The program is designed to coordinate services for eligible individuals who require significant assistance while allowing them to remain in a community setting. DenWard Healthy Living works to provide compassionate, person-centered support based on the individual's approved services and plan of care.
Structured Family Caregiver
Structured Family Caregiver (SFC) services can provide support, education, and oversight for qualifying waiver members whose family caregiver lives in the home. DenWard Healthy Living works with families to help provide a structured approach to caregiving while keeping the individual's needs, safety, dignity, and independence at the center of care.
Our SFC support may include:
- Caregiver education and guidance
- Care coordination
- Support with daily living needs
- Ongoing communication with the care team
- Assistance understanding the care plan
- Monitoring and documentation
- Resources for family caregivers

Private Home Health Services
DenWard Healthy Living also provides private home health services designed around the needs of the individual and family. Our focus is on providing dependable support while respecting each person's privacy, independence, preferences, and dignity.
